Forms Relating to Agility Training Classes

	Durango Agility Dogs Brochure - This is the DAD Brochure that you see printed in a brochure format (yellow). If you would like to print a copy or refer to information you might have seen in this brochure, you can download the brochure in .pdf format.
	DAD Training Class Registration Form - It is required that all students fill out this form (includes waiver) and turn it in the first night of class. A copy of your dog's immunization records must be presented to trainer first night of class. Click here for DOWNLOAD in .pdf format.

	DAD Waiver/Release Form - It is required that all students and DAD members who intend to participate in any training sessions offered by DAD sign this waiver/release form. This form is also included in the above Dad Training Class Enrollment Form. Click here for DOWNLOAD in .pdf format.
	DAD Training Site Parking Instructions - It is very important that we respect the property that has been so generously offered to the club as an agility training facility.
